The preliminary notice from the U.S. Department of Energy’s Office of Health, Safety and Security cited four violations in areas of hazard identification and prevention, scaffold safety, training and occupational medicine.

The accident at Savannah River Site’s K Area was followed by an investigation that “identified substantive weaknesses in SRNS’ processes to identify, control, and manage construction safety and industrial hygiene hazards,” said the notice, dated Friday and addressed to SRNS President Dwayne Wilson.

The proposed fines initially totaled $225,000, but the amount was reduced because of the contractor’s follow-up efforts.

“SRNS promptly responded to the fall injury event,” the notice said, and the contractor’s corrective actions appear to address many of the deficiencies found by federal investigators.

“As a result, the total proposed civil penalty is $159,375,” the notice said.
