According to the agency’s 41-page summary of activities from Jan. 1 to March 31, the plant workers failed to conduct a routine visual examination of a metallic liner around a reactor part; and failed to maintain proper pressure in a firewater system.
Both problems were corrected prior to the issuance of the report, and were characterized as being of very low safety significance that did not require a formal citation.
